Manejo del glaucoma neovascular refractario empleando la válvula de Ahmed. Experiencia clínica de corto plazo en un centro de referencia

RESUMEN
Propósito: Evaluar la eficacia y seguridad en el corto plazo, de la válvula de Ahmed para controlar la presión intraocular (PIO) en ojos afectados por glaucoma neovascular.
Métodos: Se analizaron retrospectivamente 521 casos de pacientes mexicanos que fueron sometidos a la implantación de una válvula de Ahmed, debido a la presencia de glaucoma neovascular congestivo. Noventa y cinco por ciento (n = 497) de los pacientes fueron seguidos al menos tres meses.
Resultados: La PIO media preoperatoria (41.3 ± 12.7 mmHg) se redujo significativamente (P = 0.0001) a los tres meses después de cirugía (17.7 ± 7.3 mmHg). El éxito global de un día, una semana, un mes y tres meses después de cirugía fue de 80.4%, 86.8%, 80.2% y 78.8%, respectivamente. El éxito quirúrgico fue más común en los casos que recibieron fotocoagulación retiniana preoperatoria (P = 0.0027). Las complicaciones más frecuentes fueron: vesícula encapsulada (13.2%), estrechamiento de la cámara anterior (9.9%), hipotonía (2.7%) e hipema (1.5%).
Conclusiones: La válvula de Ahmed es un dispositivo efectivo y seguro para controlar la PIO en pacientes mestizos mexicanos con glaucoma neovascular congestivo, al menos en el contexto del corto plazo.
