Síndrome de lipodistrofia en pacientes con infección por VIH que reciben tratamiento antirretroviral

RESUMEN
Antecedentes: el síndrome de lipodistrofia asociado al tratamiento antirretroviral en pacientes con infección por VIH se caracteriza por la pérdida subcutánea periférica y acumulación central de tejido adiposo.
Objetivo: determinar la prevalencia del síndrome de lipodistrofia en un grupo de pacientes con infección por VIH que recibía tratamiento antirretroviral en Yucatán, México.
Material y métodos: estudio prospectivo, observacional, descriptivo y transversal. Se estudiaron 123 pacientes de uno y otro sexo que acudían al Hospital General Agustín O’Horán de la Secretaría de Salud y que tenían al menos un año de recibir tratamiento antirretroviral. A cada paciente se le aplicó un cuestionario para obtener sus datos clínicos y demográficos, y se le hicieron mediciones antropométricas de los pliegues cutáneos (mejilla, bíceps, tríceps y fémur), del IMC y del índice cintura-cadera.
Resultados: la prevalencia del síndrome de lipodistrofia fue de 16%, 5% manifestó lipohipertrofia, 40% lipoatrofia y 55% un patrón mixto. Su manifestación se asoció con la duración del tratamiento antirretroviral (36.05 vs 30.7 meses, p = 0.02).
Conclusiones: la prevalencia del síndrome de lipodistrofia que se manifestó en los pacientes del estado de Yucatán fue menor a la reportada en otros países, y la antropometría ofrece una alternativa de bajo costo para diagnosticarlo en nuestro medio.
