Valoración del hipogonadismo bioquímico y clínico en hombres adultos con diabetes mellitus tipo 2

RESUMEN
Introducción: La disfunción eréctil es frecuente entre los diabéticos. La frecuencia de este problema en diabéticos mexicanos no se conoce.
Objetivo: Determinar el estado androgénico clínico y bioquímico de los pacientes con diabetes mellitus del CMN «20 de Noviembre».
Pacientesy métodos: Fueron estudiados todos los pacientes masculinos con diagnóstico de diabetes mellitus tipo 2, con edades comprendidas entre los 30 y los 75 años, que acudieron a la Consulta Externa de Endocrinología durante el período de mayo a julio de 2009. Se midieron niveles séricos de testosterona total (TT), FSH y LH, se determinó el IMC y la presencia de comorbilidades. Se aplicó el cuestionario ADAM para documentar síntomas de hipogonadismo. Se definió como hipogonadismo franco a la presencia de niveles de TT ‹ 8 nmol/L asociados a un cuestionario ADAM positivo y como hipogonadismo probable a la presencia de niveles de TT entre 8 y 12 nmol/L asociados con un cuestionario ADAM positivo.
Resultados: El 29.4% de la población de estudio presentó niveles de TT menores a 12 nmol/L asociados a síntomas de hipogonadismo. Seis pacientes (17.64%) presentaron hipogonadismo franco, mientras que 4 (11.8%) presentaron hipogonadismo probable. Seis de estos 10 pacientes tenían niveles bajos de gonadotrofinas. No se encontró asociación entre los niveles de TT y el IMC, ni con la HbA1c.
Conclusiones: El hipogonadismo es frecuente en hombres con DM tipo 2.
