Calidad y costo-efectividad en la atención del paciente crítico

RESUMEN
Resulta de gran importancia supervisar el uso de recursos en terapia intensiva, esto sin descuidar la calidad de la atención. El análisis de calidad se centra en 3 elementos: la estructura (recursos humanos, materiales e infraestructura), el proceso (acoplamiento a las guías basadas en evidencia) y los resultados (análisis de mortalidad). En cuanto al uso de recursos, los análisis económicos son de gran utilidad, ya sea minimización de costes, costo-beneficio, costo-utilidad o costo-efectividad. Todos éstos evalúan los cambios que produce una nueva estrategia en comparación a una convencional, tomando en cuenta la evolución del paciente y el uso de recursos. Así mismo, para realizar un análisis general de uso de recursos en la UTI, existe un método de cálculo relativo que puede orientar el costo–efectividad de la unidad, éste corresponde al realizado por el Dr. Rapaport en 1994 y recalibrado en el 2007. El conocimiento de indicadores que evalúen los resultados y la utilización de recursos son de gran ayuda tanto desde el punto de vista médico como administrativo, pues pueden ser utilizados para establecer prioridades en los programas de salud y elaborar recomendaciones para mejorar la eficiencia de las Unidades de Terapia Intensiva.
