Esófago de Barrett, metaplasia gástrica y la presencia de Helicobacter pylori

RESUMEN
Antecedentes: La infección por
Helicobacter pylori y su papel en el desarrollo
de esófago de Barrett, metaplasia gástrica y adenocarcinoma esofágico no ha
sido determinada en forma clara. Esta interrogante es aún mayor cuando se
consideran otros factores asociados a la infección como lo es la presencia
de inflamación y el grado de la misma.
Objetivo: describir una serie de casos
con esófago de Barrett y metaplasia gástrica así como su asociación con la
presencia de
Helicobacter pylori. Métodos: fueron revisadas las biopsias de
pacientes con esófago de Barrett y metaplasia gástrica para determinar la
presencia de infección por
Helicobacter pylori y de inflamación aguda o
crónica y el grado de la misma.
Resultados: la presencia de la infección
por
Helicobacter pylori en los casos de esófago de Barrett y metaplasia
gástrica es de 76.5% y 61.1% respectivamente. La prevalencia de la inflamación
y su gravedad también se encuentran relacionadas con la infección por
Helicobacter pylori. Conclusiones: Existe una elevada prevalencia de infección
por
Helicobacter pylori en los sujetos con esófago de Barrett y metaplasia
gástrica esofágica asociada con una mayor gravedad en el grado de inflamación.
La asociación del
Helicobacter pylori puede tener un papel en la génesis o
adyuvante en el esófago de Barrett.
