Tendencias en patología tiroidea en un centro de referencia: prevalencia estable del carcinoma papilar e incremento de hiperplasia nodular en tiroidectomías

RESUMEN
Introducción. Se han informado cambios en prevalencia de
carcinoma papilar de tiroides (CPT) en instituciones y registros
de cáncer.
Objetivo. Describir tendencias de patologías
benignas y malignas de la tiroides en un centro de referencia
de enfermedades endocrinas.
Material y métodos. Especímenes
consecutivos que contaran con preparaciones histológicas/
bloques, en el archivo de patología quirúrgica (enero
1990 a diciembre 2009). Revisión sistemática y clasificación de
los casos con documentación de tamaño, número de bloques y
tipo de escisión. Los registros institucionales consecutivos fueron
anotados, aquéllos anteriores a 1990 y sometidos a exploración
de nódulos tiroideos posterior a 1991, permitieron
calcular densidad de incidencia. Los casos se agruparon por
lustros.
Resultados. Se sometieron a escisiones 1,269 de
103,961 enfermos que recibieron registro (1.2%). Se identificaron
124 que desarrollaron nódulos posteriores al año de inicio.
La densidad de incidencia de bocio fue 0.05 persona/año y
la de CPT 0.04 por persona/año. En todo el grupo la relación
mujer/hombre fue 9:1, con mediana de 45 años de edad. En
60% se realizó tiroidectomía total o casi total. Se diagnosticó
enfermedad benigna en 732 (52%) y maligna en 678 (48%). El
número de tiroidectomías aumentó a partir de 2005 (tendencia
p = 0.03), con incremento sostenido en prevalencia de bocio
(0.25, 0.31, 0.35, 0.38, p = 0.0005), sin modificaciones
sustanciales en la prevalencia de CPT (0.41, 0.43, 0.35, 0.40, p
= 0.71).
Conclusiones. La posibilidad de identificar bocio
como único hallazgo en tiroidectomías aumentó 52% en los
últimos 20 años. La prevalencia de CPT se ha mantenido estable,
con mayor número de cirugías por tumores ‹ 3 cm.
