Nuevo concepto sobre la enfermedad de Fournier y sus fístulas

RESUMEN
Jean Alfred Fournier describió en 1883 una fascitis necrotizante del área perineal, perirrectal y genital, como una enfermedad idiopática del escroto. Actualmente la enfermedad difiere de su descripción original y se sabe estar asociada o ser secundaria a numerosas patologías y procedimientos quirúrgicos. Presentamos 12 casos con manifestaciones cutáneas de tumoración, absceso y/o fístula, en los que, la tomografía axial computada (TAC) demostró extensiones a: Periné/escroto (5-12), muslo/obturador (2-12), inguinal/espermática (2-12), abdominal-fascia transversa y fascia cuadrada (2-12) y vaina del psoas, fascia lumbodorsal, pleura parietal, intercostal interno (1-12). Los patrones de difusión se explican por la anatomía de las fascias de la pelvis, periné, abdomen y tórax. Las modalidades de imagen por TAC demostraron su utilidad para confirmar la enfermedad, evaluar su extensión, determinar su etiología y planear su tratamiento quirúrgico. El tratamiento quirúrgico consistió en debridación, necrectomía, fasciectomía, canalización con Penroses, además de antibióticos y medidas de apoyo. Con excepción de dos fístulas perineales con cierre espontáneo en 8 y 12 meses, y fallecimiento de un anciano diabético, septicémico con fístula torácica, la evolución de los pacientes fue favorable. No obstante, se requieren estrategias adicionales para mejorar la terapéutica quirúrgica.
