Sigmoidectomy because of bleeding ulcer by CMV infection in transplanted patient. A clinical case presentation

ABSTRACT
Introduction. The infection for cytomegalovirus (CMV), is one of the main causes of morbi-mortality in the transplanted patient.
Clinical case. Masculine of 49 years with renal transplant 72 days ago, he presents liquid evacuations and hemorrhages digestive. Positive antigenemia for CMV. Ganciclovir began. Endoscopic low: dug lession in oval way of 4 cm in sigmoid. Three days later, he presents hemorrhages digestive again, for what is carried resection of sigmoid for bleeding ulcer.
Histopatologic: Active chronic colitis ulcerated by cyitomegalovirus.
Discussion. The digestive affectation, is second in frequency after the lung one, it is a process erosive or ulcerative that it studies with a clinical presentation that is keeps relationship with the affected area. The affectation in colon is characterized by watery diarrhea or with blood.
Conclusion. Therefore in transplanted patients, when it exists diarrhea and hemorrhages digestive it should be suspected in infection by CMV and it should be begun specific treatment as soon as possible Ganciclovir that have effectiveness of more than 80% and in extreme cases the surgical intervention should be carried out.
