Angioplastía con stent en el tratamiento de coartaciones aórticas críticas

RESUMEN

Objetivo: Dar a conocer nuestra experiencia en el tratamiento endovascular de la coartación de la aorta de importante repercusión hemodinámica. Métodos: De mayo de 2000 a febrero de 2006, se estudiaron 7 pacientes, con el diagnóstico de coartación aórtica. A todos ellos previo consentimiento se les realizó historia clínica, electrocardiograma, radiografía de tórax, ecocardiograma Doppler color transtorácico y estudio angiográfico. Todos los pacientes estaban hipertensos y recibían 1 ó 2 fármacos para su control. Resultados: Fueron 7 pacientes tratados con angioplastía y stent P4014; 6 hombres y 1 mujer; el promedio de edad fue 24.14 ± 8.57 años. El gradiente promedio transcoartación fue de 80 mm Hg. El gradiente residual posterior al tratamiento endovascular fue de 4 mm Hg. Se encontró insuficiencia aórtica en 4 de los 7 pacientes que mejoró notablemente después del tratamiento, así como la hipertensión arterial; quedando sólo 3 pacientes con 1 solo fármaco para su control. Conclusiones: A pesar de ser una serie pequeña, creemos necesario predilatar la lesión antes de liberar un stent en coartaciones críticas.
