Síndrome TINU: una entidad probablemente infradiagnosticada

RESUMEN

El síndrome TINU (tubulointerstitial nephritis and uveitis) es una rara entidad descrita por primera vez en 1975 por Dobrin et al.(1) Desde ese momento, se han descrito en la literatura unos 300 casos.(2) Típicamente afecta a edad infantil y adolescentes con predominio claro en la mujer (9:1 en algún estudio). Dicha enfermedad consiste en la asociación de síndrome inflamatorio sistémico, con aumento de reactantes de fase aguda, nefropatía túbulointersticial proximal (glucosuria, leucocituria) y uveítis anterior bilateral no granulomatosa, en la mayoría de los casos. A continuación presentamos un caso de dicha entidad con evolución y respuesta terapéutica favorables. Los síntomas iniciales fueron inespecíficos, pero presentó deterioro importante de la función renal, por lo cual se realizó tratamiento esteroideo con respuesta exitosa y sin nuevas recaídas durante el seguimiento.
