Riesgo de persistencia postratamiento de la neoplasia intraepitelial cervical con infección por VPH 16

RESUMEN

Introducción: El proceso de oncogénesis para el desarrollo de lesiones cervicales de alto grado requiere de la infección persistente de un virus del papiloma humano de alto riesgo, entre los cuales, el VPH16 es el subtipo con menor tasa de aclaramiento.
Objetivo: Determinar si la infección VPH16 previa a la conización constituye un factor de riesgo para la persistencia de lesión precursora tras tratamiento.
Métodos: En febrero de 2020 se realiza una revisión sistemática sobre la persistencia de enfermedad tras LEEP/LLETZ en pacientes con infección de VPH16 previo al tratamiento. Se procede a una búsqueda en las bases de datos PubMed-Medline del National Center of Biotechnology Information (NCBI) y ClinicalTrial.gov de la National Library of Medicine, ambos en EE.UU. Se obtuvieron 418 artículos, de los cuales fueron elegibles 8, todos ellos de cohorte retrospectiva. En cuatro artículos se encontró una relación significativa entre la presencia de infección con VPH16 en los controles previos a la conización, y el riesgo de fallo de tratamiento; en los cuatro restantes no se encontró esta relación.
Conclusiones: En la literatura no existe unanimidad sobre si la infección de VPH16 previo al tratamiento es un factor independiente de persistencia de lesión. Se requieren más estudios, con una mayor población para determinar si el VPH16 positivo previo al tratamiento sirve para identificar un grupo de riesgo de fallo de tratamiento.
