Reconstrucción de párpado tras resección de carcinomas cutáneos

RESUMEN
ANTECEDENTES: La resección de carcinomas cutáneos en
párpados, principalmente carcinoma basocelular (CBC) y
carcinoma epidermoide (CEC), puede dejar defectos anatómicos
que requieren reconstrucción especializada. La
restauración funcional y estética del párpado es fundamental
para proteger el globo ocular y mantener la simetría
facial.
MATERIAL Y MÉTODO: Se realizó una revisión narrativa de la
literatura sobre las técnicas quirúrgicas empleadas para
la reconstrucción palpebral tras resección de carcinomas
cutáneos. Se analizaron estrategias según localización,
tamaño y espesor del defecto, diferenciando abordajes para
párpado superior e inferior.
RESULTADOS: El cierre primario es viable para defectos
menores del 25%. Para defectos entre 25% y 67%, se
emplean colgajos locales como los de Tenzel, Mustardé o
Tripier. Defectos mayores requieren técnicas en dos tiempos
como Hughes o Cutler–Beard, o colgajos regionales
como el de Fricke. Las reconstrucciones exitosas dependen
de la adecuada selección técnica según el defecto y
la preservación funcional para evitar complicaciones como
ectropión, retracción o lagoftalmos.
CONCLUSIONES: La reconstrucción palpebral tras resección
oncológica exige planificación individualizada. La combinación
de conocimiento anatómico, selección adecuada de técnicas y seguimiento postoperatorio permite restaurar
tanto la función ocular como la estética periocular.
