Medicamentos potencialmente inapropiados en el adulto mayor en servicios de urgencias: una revisión narrativa

RESUMEN
La prescripción de medicamentos potencialmente inapropiados en adultos mayores representa un desafío crítico en los servicios de urgencias,
donde los cambios fisiológicos propios del envejecimiento, junto con la polifarmacia, incrementan el riesgo de desenlaces adversos. En
esta revisión narrativa se sintetiza evidencia científica de metaanálisis, revisiones sistemáticas y guías estandarizadas internacionales publicadas
en la última década con el fin de identificar los medicamentos potencialmente inapropiados más frecuentemente usados en urgencias en
población geriátrica. El análisis revela que el uso de medicamentos como benzodiacepinas, AINEs, algunos antihistamínicos, antipsicóticos,
antihipertensivos o hipoglucemiantes en urgencias se asocian con mayor incidencia de delirium, caídas y deterioro de la función renal; en
algunos casos, también prolongan la estancia hospitalaria y elevan el riesgo de mortalidad.
La identificación de estos medicamentos, la individualización del paciente y la transición a alternativas más seguras son medidas necesarias
para garantizar la seguridad del paciente, reducir los tiempos de estancia hospitalaria, y mejorar el uso racional de medicamentos en el área de urgencias.
