Platelet transfusion refractoriness: diagnosis and management

Introduction
Platelet transfusion refractoriness is a repeated
suboptimal response to platelet transfusion,
manifested by lower-than-expected posttransfusion
platelet count increments. The first
tasks are to confirm that the patient is truly
refractory and to determine whether the cause
is non-immune, immune, or a combination of
both. Non-immune factors account for most
cases, whereas immune-mediated refractoriness
is less common but requires specialized testing
and platelet selection.
