Nueva clasificación para la luxación traumática de la cadera

RESUMEN

Se realizó un estudio retrospectivo y prospectivo en el Hospital ABC, entre enero de 1992 y agosto de 1998, en 37 pacientes con luxación-fractura de la cadera. Se propone una nueva clasificación para estas lesiones que incluye cinco tipos, basada en la estabilidad y complejidad de la fractura acetabular de acuerdo con los criterios de Letournel, diferenciando a la luxación pura como tipo I y como una entidad distinta a las luxaciones-fracturas simples, las luxaciones asociadas a fracturas elementales estables como tipo II, las asociadas a fracturas elementales inestables, tipo III, a fracturas complejas tipo IV y a fractura de la cabeza femoral como tipo V. El tratamiento incluyó reducción cerrada en los casos de luxación pura y luxaciones-fracturas simples, y para la mayoría de las fracturas complejas reducción abierta y osteosíntesis. Las lesiones se clasificaron de acuerdo con el sistema propuesto y los resultados se describen de acuerdo con los criterios de Merle d’Aubigné y Postel, modificados por Matta en excelentes, buenos, regulares y malos. Se presenta un seguimiento de hasta seis años. Se encontraron mejores resultados en pacientes con luxaciones puras y con luxación-fractura simple. La clasificación propuesta tiene valor pronóstico y terapéutico.
