RESUMEN

Revisamos los resultados en 82 pacientes con fractura de la diáfisis femoral entre 0-13 años de edad, tratados conservadoramente bajo dos esquemas: 1) Aparato de Yeso Tóraco-pélvico-pedio previa tracción esquelética, dependiendo del desplazamiento inicial al momento del ingreso a nuestro Hospital. El seguimiento se hizo en consulta externa con un promedio mínimo de 32 semanas y control radiográfico frecuente por las primeras semanas. Los resultados en pacientes menores de 6 años fueron similares en ambos grupos, ninguno con acortamiento mayor de 2cm ni deformidad angular mayor de 15°. En los pacientes mayores de 6 años obtuvimos en el grupo Tóraco-pélvico-pedio inicial sólo 8 pacientes (16%) con deformidad angular mayor de 15°; del grupo de Tóraco-pélvico-pedio previa tracción sólo 2 pacientes con acortamiento mayor de 2 cm y 18 pacientes con angulación mayor de 15° (54%). El tratamiento conservador, dependiendo del desplazamiento inicial, en pacientes menores de 6 años obtiene resultados excelentes similares en ambos grupos; los resultados no deseables en acortamiento y angulación se obtuvieron en pacientes mayores de 6 años donde se sugiere diferente tipo de tratamiento.
