Sepsis en el niño quemado: Características clínicas y microbiológicas en una serie retrospectiva

RESUMEN
Introducción: Las quemaduras en el niño representan un grave problema de salud pública en el mundo. La sepsis es la principal causa de muerte. El objetivo del presente estudio es conocer el comportamiento clínico y hallazgos bacteriológicos de niños quemados, con sepsis.
Métodos: Se revisó una serie de casos retrospectiva de 19 niños con sepsis asociada a quemaduras, en un periodo de 17 meses; analizando variables sociodemográficas, tipo y extensión de la lesión, signos clínicos y hallazgos bacteriológicos. Se utilizó estadística descriptiva para la presentación de los resultados.
Resultados: Las escaldaduras fueron la causa principal de las quemaduras (84.2%). El porcentaje de la superficie corporal afectada fue del 23.4% en promedio; la mayoría de las lesiones fueron de espesor parcial. Las manifestaciones iniciales de sepsis fueron: incremento en la taquipnea y alteraciones gastrointestinales (94.7%); un cambio súbito en la temperatura predominantemente ascenso, también fue un dato constante (89.4%). El 73.7% presentó la sepsis entre los 6 y 15 días posteriores a la quemadura. De los microorganismos causales, los Gram-negativos representaron el 72.4%.
Klebsiella sp.,
Staphylococcus coagulasa negativa y
Citrobacter sp., fueron los gérmenes con mayor frecuencia de aislamiento. Fallecieron 5 niños.
Conclusiones: La sospecha clínica de sepsis en el niño quemado apoya el inicio de tratamiento antimicrobiano empírico, previa toma de cultivos.
