RESUMEN

El síndrome de infusión de propofol (SIP) es una rara, pero potencialmente letal complicación asociada al uso de infusiones de propofol a dosis elevadas y por tiempo prolongado. Se ha correlacionado en la mayoría de los estudios a dosis de propofol mayores de 4 mg/kg/h por más de 48 horas. El SIP se caracteriza por disfunción multiorgánica, rabdomiólisis, acidosis metabólica, hipercaliemia, arritmias y muerte cardíaca súbita. Es secundario a alteraciones en el metabolismo oxidativo mitocondrial de ácidos grasos de cadena larga. El tratamiento del SIP consiste en la suspensión del medicamento, apoyo cardiovascular, corrección de la rabdomiólisis, acidosis láctica e hipercaliemia.
