RESUMEN

Se comunica el caso clínico de una paciente femenina de 16 años de edad que ingresó por politraumatismo secundario a un accidente automovilístico. La paciente presentaba escalpe frontal y fractura lineal del cráneo, en el fémur izquierdo y la rama isquiopúbica derecha. Se colocó un clavo centromedular femoral y se reparó el escalpe frontal, la fractura isquiopúbica y de cráneo se atendieron de manera conservadora. El diagnóstico se estableció con la exclusión de otras causas del cuadro clínico. Se le indicó ventilación no invasora con IPAP de 9 cmH20 y EPAP de 4 cmH20 con lo que la enferma tuvo mejoría significativa en las primeras ocho horas.
