Luxación lumbosacra unilateral inveterada en niños. Caso clínico y revisión de literatura

RESUMEN

La luxación facetaria lumbosacra asociada o no a fractura es una lesión poco frecuente en niños, siendo el mecanismo la hiperflexión por traumatismos de alta energía. Presentamos el caso de una femenina de 8 años que inició su padecimiento al caerle una portería de fútbol directamente en área lumbar, atendiéndose en urgencias con diagnóstico de esguince lumbar. Al tercer mes acude a urgencias por persistencia de dolor, deformidad lumbosacra y limitación funcional. La radiografía AP y lateral de columna demostró espondilolistesis traumática secundaria a luxación facetaria y fractura de apófisis transversas de L4 y L5. El examen neurológico fue normal. Se realizó instrumentación con artrodesis posterolateral L4-S1, con adecuada evolución postoperatoria. Se logró adecuada fusión a los 3 meses estando la paciente asintomática a 13 meses del postoperatorio.
