RESUMEN

La trombocitopenia inducida por heparina (TIH) ocurre en el 5-10% de los enfermos tratados con heparina y tiene dos formas de presentación; la forma grave es causada por la presencia de anticuerpos dependientes de heparina provocando agregación plaquetaria y posible trombosis. La TIH con trombosis (TIHT), aunque rara (0.2-0.4%), empeora el pronóstico e implica la inmediata suspensión de la heparina, el tratamiento de las complicaciones y la adecuada elección de la alternativa terapéutica cuando se precise continuar la anticoagulación. El diagnóstico se basa en criterios clínicos y puede apoyarse mediante pruebas de laboratorio diseñadas para detectar los anticuerpos dependientes de heparina.
