Procalcitonina sérica como factor pronóstico de apendicitis perforada en niños

RESUMEN

El diagnóstico de apendicitis en niños, en ocasiones puede ser muy difícil, hasta el momento no existe ninguna prueba confiable para predecir alguna complicación de la misma.
Los pacientes con apendicitis perforada tienen un curso clínico distinto a los pacientes con apendicitis no complicada: requieren mayor cobertura antibiótica, más días de hospitalización y están expuestos a mucho mayor número de complicaciones. Además, la apendicitis complicada con plastrón o absceso se puede tratar en forma conservadora, lo cual disminuye la frecuencia de complicaciones transoperatorias. La procalcitonina (PCT) es un marcador sérico de inflamación, confiable, utilizado en pacientes con sepsis, procesos inflamatorios y en algunos procedimientos quirúrgicos como trasplante de órganos y cirugía cardiotorácica. Si la procalcitonina sérica fuese confiable para pronosticar apendicitis perforada, se podría utilizar sistemáticamente al momento del diagnóstico para determinar el plan terapéutico más adecuado para el paciente y conocer el pronóstico del padecimiento desde la evaluación inicial.
