Fascitis Necrotizante Perineal: Gangrena de Fournier

RESUMEN

La fascitis necrotizante es una infección progresiva y potencialmente fatal de los tejidos subcutáneos y la piel; cuando ésta afecta el periné, el escroto o el área perianal se le conoce como Gangrena de Fournier (GF). Se presenta frecuentemente en pacientes con enfermedades que comprometen el sistema inmune (diabetes mellitus, alcoholismo o cáncer) condicionando un avance rápido del proceso infeccioso y su extensión a la pared abdominal y/o a los miembros pélvicos. El tratamiento integral debe incluir la debridación agresiva y extensa de la piel y los tejidos subcutáneos infectados, así como soporte hemodinámico, metabólico y de antibióticos apropiado; sin embargo, a pesar de las mejores medidas terapéuticas, en la actualidad su mortalidad es del 20 al 40%.
