Flujo salival y prevalencia de xerostomía en pacientes geriátricos

RESUMEN
Objetivos: Valorar el flujo salival total basal y estimulado, así como la prevalencia de xerostomía, estimar la asociación entre la saliva basal y la percepción de xerostomía en pacientes geriátricos sanos, con diabetes mellitus y/o hipertensión arterial.
Diseño del estudio: Se seleccionaron 440 pacientes, que fueron divididos en cuatro grupos por su condición de salud. Se empleó la prueba de sialometría para medir el flujo salival basal y estimulado a partir de la saliva total y se cuantificó mediante análisis gravimétrico. Se diseñó un cuestionario para evaluar la presencia de xerostomía, signos y síntomas asociados a ella.
Resultados: El flujo salival basal de los diabéticos con hipertensión fue significativamente menor que en las personas sanas (p = 0.023). El flujo salival estimulado por la masticación en los pacientes con diabetes y/o hipertensión fue significativamente diferente con respecto a los sanos (p ‹0.001). La prevalencia de xerostomía en los sujetos diabéticos y/o hipertensos fue mayor comparada con los sanos (p ‹0.001). Los pacientes con xerostomía tuvieron los niveles más bajos de saliva basal comparado con los que no sintieron la boca seca (p‹0.001). Los sujetos con xerostomía manifestaron dificultad para pasar algunos alimentos, dificultad para hablar por falta de saliva, resequedad en la garganta y necesidad de tomar agua para masticar alimentos secos. En estas personas el flujo salival basal y estimulado fue menor.
Conclusiones: Los pacientes con diabetes e hipertensión de nuestro estudio, presentan los niveles más bajos de flujo salival y la tasa más alta de xerostomía, en comparación con los sujetos sanos, lo que sugiere que la secreción salival y la prevalencia de xerostomía son afectadas por la condición médica de los sujetos.
