Tratamiento endoscópico de la estenosis subglótica subaguda mediante el uso de la férula endotraqueal tipo Freitag (stent dinámico)

RESUMEN

Se presenta el caso de una paciente de 42 años de edad, portadora de estenosis subglótica benigna secundaria a intubación prolongada y traqueotomía cruenta, que ameritó cierre quirúrgico del estoma traqueal, desarrollando un área estenótica del tercero al quinto anillos traqueales. La evaluación funcional inicial mediante curva flujo-volumen completa mostró la presencia de obstrucción fija de vías aéreas extratorácicas. La estenosis fue resecada endoscópicamente con electrocauterio, colocando de inmediato tubo en T de Montgomery durante 6 meses, mismo que se reemplazó por una férula endotraqueal tipo Freitag durante 3 meses más para permitir cerrar al traqueostoma. Al retirar esta férula se observó una reepitelización normal y una luz traqueal del 80%, sin malacia. La curva flujo-volumen final mostró obstrucción variable de vías aéreas extratorácicas y mejoría de la capacidad vital forzada y de los flujos espiratorios.
