Resultados inmediatos y a mediano plazo postimplante de stents coronarios en pacientes diabéticos

RESUMEN

Revisamos nuestra experiencia y evaluamos los resultados clínicos y angiográficos así como el seguimiento de los pacientes diabéticos que fueron sometidos a angioplastía coronaria transluminal percutánea (ACTP) con colocación de prótesis endovasculares “stents”. Material y métodos: En el período comprendido de enero de 1995 a diciembre de 1999 durante el cual se realizaron 896 procedimientos, a 770 pacientes, de éstos 137 además con diagnóstico de diabetes mellitus (DM) con 160 férulas endovasculares con (1.16 stents por paciente), con edades de 35 a 80 años promedio de 58.70 ± 8.44 con 92 hombres y 45 mujeres, representan el 17.7% del total de esta población. Con angina severa inestable según la Sociedad Canadiense de Cardiología (SCC) 54%, con antecedentes de infarto del miocardio 62% y enfermedad multivascular 68%, el porcentaje promedio de obstrucción vascular fue de 90 ± 6.3 y la fracción de expulsión rangos de 40 a 65% promedio 45.3 ± 8.7. Resultados: El éxito angiográfico inmediato, 94.4%. Las complicaciones: infarto agudo del miocardio y trombosis subaguda 8.4%. La mortalidad 4.3%. El seguimiento fue de seis a 36 meses promedio de 10 meses fue obtenido en 94%. Conclusiones: Estos resultados sugieren que la aplicación de stents coronarios en DM es un procedimiento exitoso clínico y angiográfico con bajo porcentaje de complicaciones a pesar de su mayor perfil de riesgo.
