RESUMEN

Introducciòn: Neoplasia maligna y extremadamente agresiva, el tumor rabdoide teratoide atípico representa de 1 a 2% de los tumores del SNC de pacientes pediátricos. Esta entidad era clasificada dentro de los teratomas, sin embargo, se ha determinado que a diferencia de éstos, presenta en inmunohistoquímica positividad para vimentina, antígeno de membrana epitelial y proteína acida fibrilar glial.
Casos Clínicos: Presentamos dos casos de pacientes con tumor teratoide rabdoide atipico que fueron sometidos a manejo neuroquirúrgico y posterior quimioterapia y radioterapia en el Hospital Juárez de México. Paciente 1: Masculino de 11 meses, con antecedente de hidrocefalia, se colocó derivación ventriculoperitoneal. Ingresó por disfunción valvular, se realizó recambio valvular y craniectomía suboccipital. Posteriormente se realizó abordaje supra e infratentorial y resección parcial de la lesión. Se envió a quimioterapia. La sobrevida fue de cinco meses. Paciente 2: Masculino de ocho años acudió por cefalea de dos semanas de evolución, náuseas, vómito y hemiparesia izquierda, se hizo estudio de imagen que mostró lesión parietal derecha gigante. Se realizó craniectomia fronto-parieto-temporal derecha, para la resección parcial de la lesión y se envió a quimioterapia y radioterapia.
Conclusion:El tumor rabdoide teratoide atípico es una entidad relativamente nueva, con pocos casos descritos en la literatura médica, es extremadamente agresivo y con mal pronóstico a corto plazo. Reportamos dos pacientes con esta lesión que fueron sometidos a resección, sin embargo, por las características infiltrativas, el tamaño de la lesión al momento de diagnóstico y su vascularización tan importante fue imposible su resección total.
