Laparoscopia en trauma abdominal

RESUMEN

Antecedentes: Recientemente se ha despertado el interés en las indicaciones de las técnicas laparoscópicas en la evaluación del trauma abdominal. Métodos: Entre julio 1993 a marzo de 2005, se estudiaron prospectivamente 228 pacientes con traumatismo abdominal que fueron programados para celiotomía basándonos en la evaluación clínica. Masculinos 90.3%, femeninos 9.7%, edad 29.3 ± 3.6 años, que fueron sometidos a laparoscopia diagnóstica antes de realizar la celiotomía exploradora. Resultados: Las complicaciones ocurrieron en el 3.50%. Ocho de los pacientes con trauma penetrante localizado en la pared anterolateral del abdomen no tenían evidencia de penetración a la laparoscopia, y no se comprobó la existencia de lesiones a la celiotomía. Conclusiones: La laparoscopia diagnóstica resultó segura en el manejo de trauma abdominal. Con los recursos tecnológicos actualmente disponibles la laparoscopia diagnóstica se ha hecho más segura y eficaz en la evaluación de las lesiones dudosas. Aunque, existen áreas de la cavidad abdominal que no pueden ser visualizadas adecuadamente a la laparoscopia.
