Necrosis de gran extremidad secundaria a síndrome antifosfolipídico catastrófico

RESUMEN

La denominación de síndrome antifosfolipídico “catastrófico” define una forma grave y rápidamente evolutiva de síndrome antifosfolipídico que conduce a insuficiencia multiorgánica y muerte. Todos los pacientes que lo padecen sufren accidentes oclusivos en un corto periodo (días a semanas), el índice de mortalidad es de 50%. Su tratamiento incluye glucocorticoides, heparina, recambio plasmático, ciclofosfamida y gammaglobulinas intravenosas. Comunicamos el caso de un paciente con cuadro de rápida evolución, caracterizado por afectación multiorgánica, trombosis y posterior necrosis aséptica de la extremidad inferior derecha; el paciente sobrevivió gracias al tratamiento temprano y multidisciplinario, aunque terminó perdiendo la extremidad afectada.
