Eficacia del doble abordaje en la enfermedad de Scheuermann. Descripción de casos clínicos

RESUMEN

Objetivo: Mostrar nuestra experiencia en el tratamiento quirúrgico de la enfermedad de Scheuermann con indicación quirúrgica. Material y métodos: Se presenta una serie descriptiva, retrospectiva de 15 pacientes con enfermedad de Scheuermann en el período comprendido de 1994 a 2001, en los que estaba indicado el tratamiento quirúrgico. Se utilizó el método de Cobb para medir los grados de deformidad cifótica, que se efectuó en el preoperatorio, postoperatorio inmediato y a los dos años de la cirugía. La técnica quirúrgica realizada en todos los pacientes fue una combinación de liberación anterior con una instrumentación y artrodesis posterior. En el postoperatorio se utilizó corsé de yeso. Resultados: De 15 pacientes estudiados, todos sexo masculino, márgenes de edad 9 a 45 años (17 ± 8.15), con cifosis inicial de 68° a 100°( 79.53 ± 10.46), la cifosis postquirúrgica inmediata fue de 32.73° ± 10.10. Con corrección general de 45.6° ± 16.78 con una p ‹ .005; El corsé de yeso se usó en promedio 11.4 meses después de la cirugía. El 40% presentó complicaciones tardías y 27% ameritó retiro de las barras. La cifosis a dos años fue de 37.06° ± 9.69 con una pérdida de corrección de 4.6° p ›; .05. Conclusiones: En nuestra experiencia con enfermedad de Scheuermann que amerita tratamiento quirúrgico en los que se logra una adecuada corrección de la cifosis, hemos observado pérdida de corrección no significativa a los dos años.
