Utilidad del metilmetacrilato en el tumor de células gigantes óseo

RESUMEN

El tumor de células gigantes óseo es una neoplasia con potencial maligno, alto índice de recurrencia y posibilidad de metástasis pulmonares, especialmente cuando es manejado en forma inadecuada y/o con radioterapia. Realizamos un estudio retrospectivo, observacional, longitudinal y descriptivo en 35 pacientes en edades de 15 a 75 años, mayormente femeninos a razón de 1.3:1. El sitio principalmente afectado fue el tercio distal del fémur y proximal de la tibia con 80% de los casos. Se practicó resección intracapsular con aporte de metil-metacrilato en 31 y resección marginal con aporte de metil-metacrilato sostenido con tornillos en cuatro casos. Se presentó recidiva local de la lesión en nueve (25.7%) entre 6 y 16 meses del postoperatorio, manejados con artrodesis de la rodilla o artroplastía de sustitución con prótesis no convencional. Un paciente sufrió metástasis pulmonares, por las cuales falleció a los 36 meses del manejo de la recidiva local. El resultado en base a los criterios de la Sociedad Internacional de Tumores del Sistema Musculoesquelético, fue excelente en 27 (77.1%) y bueno en ocho (22.9%). El aporte de metil-metacrilato, tras la recepción intracapsular o marginal de la lesión, constituye una alternativa efectiva, económica, versátil y segura, que permite el uso de otras alternativas terapéuticas en caso de recidiva.
