RESUMEN

Introducción: El quiste de colédoco, considerado una patología benigna, se asocia con malignización, colangitis, pancreatitis y colelitiasis. La nueva tecnología en imagen y cirugía ha generado controversia en cuanto al método diagnóstico y quirúrgico de elección. Objetivo: Realizar una revisión de la literatura mundial de los últimos cinco años para —con base en evidencia— uniformar criterios y mejorar el pronóstico. Material y métodos: Revisión sistemática en bases de datos electrónicas (Medline, Ovid y Elsevier ClinicalKey) utilizando el término “choledochal cyst” de artículos publicados en inglés entre enero de 2009 y agosto de 2014. Se clasificaron para su revisión según su enfoque principal (epidemiología, clasificación, presentación, método diagnóstico, patología, tratamiento y pronóstico). Se comparó la información y reportó de manera objetiva cada uno. Resultados: Se encontraron 64 artículos. Según el enfoque principal: presentación (4), diagnóstico (12), patología (11), tratamiento (28) y pronóstico (9); todos abordaban epidemiología con datos similares. Conclusiones: El quiste de colédoco es una entidad rara que requiere resección para evitar complicaciones. La hepático-duodeno anastomosis se asocia a más complicaciones. La resección laparoscópica es segura en manos expertas. Se requieren más estudios sobre resección con robótica en pediatría. La resección no reduce el riesgo de malignización a niveles basales, por lo que la vigilancia debe prolongarse.
