Oftalmía Simpática

RESUMEN

La Oftalmía Simpática es una uveítis bilateral rara y potencialmente devastadora, típicamente seguida de un trauma ocular penetrante o una cirugía intraocular. Un alto índice de sospecha es vital para asegurar diagnóstico y tratamiento temprano para obtener buen resultado visual. El tiempo de inicio varía de días a décadas, con el 80% de los casos iniciando antes de los 3 meses y el 90% dentro del primer año. La presentación clínica puede ser muy variable, con la presencia de inyección conjuntival, depósitos retroqueráticos en grasa de carnero, desprendimiento seroso de retina, edema de papila y los característicos nódulos de Dalen-Fuchs. El tratamiento tiene como pilar los esteroides sistémicos, pudiendo utilizar inmunomoduladores en casos refractarios. Se presenta caso de un joven que sufre evisceración secundaria a traumatismo ocular penetrante, que días posteriores desarrolla un cuadro de Oftalmía Simpática en ojo contralateral, con identificación y tratamiento temprano.
