Síndrome de Mirizzi

RESUMEN

El Síndrome de Mirizzi descrito en 1948 por el cirujano Pablo Luis Mirizzi, es una complicación poco frecuente que ocurre en aproximadamente 1% de los pacientes con colelitiasis.
Consiste en la impactación de un lito biliar a nivel del conducto cístico o infundíbulo, que posteriormente produce una compresión del conducto hepático común, pudiendo ocasionar erosión y generar una fístula colecisto-coledociana.
Clínicamente cursa con ictericia de origen obstructivo, y síntomas comunes de patología biliar.
El diagnóstico preoperatorio sigue siendo de gran dificultad, diversos estudios han sido utilizados, entre los cuales se encuentran; la ecografía abdominal, la colangiopancreatografía endoscópica retrógrada, la colangiografía percutánea o la colangiorresonancia, según disponibilidad en los centros hospitalarios.
El tratamiento sigue siendo quirúrgico, el abordaje operatorio, puede ser vía abierta o laparoscópica, en función del estadio y la experiencia del cirujano a cargo.
