Psoraleno tópico de uso nocturno versus uso diurno para disminuir eventos adversos en pacientes con vitiligo

Introducción: El vitiligo es una alteración melanocitopénica adquirida que afecta a 2% de la población. El tratamiento con psoralenos tópicos favorece la repigmentación de la piel pero con riesgo de fototoxicidad.
Objetivo: Determinar la seguridad y efectividad de la aplicación nocturna comparada con la diurna de 8-metoxipsoraleno en vitiligo.
Material y métodos: Se realizó un ensayo clínico aleatorizado, simple ciego en pacientes con vitiligo, los cuales recibieron 8-metoxipsoraleno vía tópica cada 24 horas durante 12 semanas. El grupo 1 lo aplicó por las noches, con exposición solar diurna previo aseo de la piel afectada; mientras que el grupo 2 lo aplicó por las mañanas y 30 minutos después se expuso a la luz solar entre las 10 y 16 horas durante un minuto, incrementando el tiempo y frecuencia a tolerancia.
Resultados: Se incluyeron 37 pacientes con vitiligo. La fototoxicidad se presentó en un paciente del grupo 1 (5.3%) y en ocho pacientes (44.4%) del grupo 2. La repigmentación parcial fue similar en ambos grupos, ningún paciente logró una repigmentación total.
Conclusiones: El presente estudio muestra que la aplicación nocturna de 8-metoxipsoraleno disminuye el riesgo de eventos adversos sin comprometer la eficacia del tratamiento.
