El problema

La incidencia de muy bajo peso al nacer (‹ 1 500 g) varía de 0.6 a 3% de todos los nacimientos. El grupo Neosano en México, que es una red de hospitales en la Ciudad de México y Oaxaca, detectó una prevalencia de muy bajo peso al nacer de 1.4% entre 29 987 nacimientos registrados por hospitales (privados y públicos) participantes, durante el período 2002-2004. De los 130 millones de bebés que nacen al año en el mundo, puede haber de 780 000 a 3 900 000 recién nacidos de muy bajo peso (RNMBP). Este grupo de neonatos es un reto para los neonatólogos que luchan por mejorar la supervivencia y la calidad de vida de estos “pequeños gigantes”, que es como los llamamos en el Hospital Infantil de México Federico Gómez, por el “pequeño tamaño” y por la “gigante” fortaleza que muestran ante la adversidad del mundo extrauterino. Desde un punto de vista global, es difícil conocer la supervivencia de este grupo de neonatos, ya que en muchas regiones no se documentan estos nacimientos. Todo esto es secundario a un fatalismo y a una aceptación de la alta mortalidad de este grupo de neonatos, especialmente en países en desarrollo, que es donde se encuentra 99% de las muertes neonatales.
