RESUMEN

Introducción: El dolor de columna no traumático es usualmente de tipo mixto con contractura muscular añadida.
Objetivo: Evaluar la eficacia y tolerabilidad del tratamiento del dolor no traumático de columna con la combinación diclofenaco, complejo B y el miorrelajante tiocolchicósido.
Métodos: Estudio doble ciego, controlado, multicéntrico, aleatorizado. Al grupo 1 se le aplicó tiocolchicósido, diclofenaco y vitaminas B y al grupo 2, diclofenaco y vitaminas B. La dosis fue 1 tableta cada 12 horas por 48 horas. Evaluaciones: Evolución del dolor, grado de alivio del dolor, contractura y funcionabilidad, distancia dedo piso, satisfacción global médico y paciente, necesidad de continuidad del tratamiento y reacciones adversas.
Resultados: Se analizaron 148 pacientes: 74 del grupo 1 y 74 del grupo 2. La disminución y grado de alivio del dolor, reducción de la contractura y satisfacción global paciente e investigador fueron estadísticamente significativos a favor del grupo 1. La valoración de la funcionabilidad y la movilidad en lumbalgias fueron mejores en el grupo 1 sin diferencia estadísticamente significativa. A las 48 horas el 93,2 % del grupo 1 y el 79,2 % del grupo 2 dejaron el tratamiento por alivio suficiente del dolor. Las reacciones adversas fueron mínimas en ambos grupos.
Conclusiones: El tratamiento con tiocolchicósido, diclofenaco y vitaminas B es más eficaz y rápido para lograr un efecto analgésico que con diclofenaco y complejo B. Se demuestra la validez de añadir un miorrelajante en un tratamiento con antinflamatorio y vitaminas B.
