Third trimester intraligamentary ectopic pregnancy

ABSTRACT

Introduction: Intraligamentary pregnancy is a rare form of abdominal ectopic pregnancy. Rarely, it comes to term, and a good condition newborn is obtained. Presurgical diagnosis is complicated.
Objective: To describe the clinical case of a patient with 31 weeks intraligamentary pregnancy, with an alive newborn.
Case report: A 29-year-old patient, with 31 weeks of gestation, came to our clinic referred from her health area. She was diagnosed with intrauterine growth retardation, oligoamnios and a placental image compatible with chorioangioma, as well as history of elevated alpha-fetoprotein, a twin pregnancy with loss of one of the fetuses, and the presence of a double uterus. Ultrasound revealed a large placental tumor, highly vascularized and compression of fetal parts, 69.6MoM alpha-fetoprotein and BHCG: 3281 u/I indicative of possible choriocarcinoma with no evidence of metastasis. Laparotomy was performed, the finding of which was intraligamentary ectopic pregnancy with a live male fetus of 1300 g, Apgar 9/9, and deformed fetal parts. The histopathological report found that the placenta was compatible with ligament ectopic pregnancy of the third trimester.
Conclusions: The diagnosis of ectopic pregnancy in the broad ligament is rare, as well as the absence of symptoms. In this case, it could not be established by ultrasonography prior to surgery, nor was it possible to establish that the diagnosed placental tumour was the product of ectopic pregnancy or related to a chorioangioma, since the histological results were not conclusive.
