Leiomyosarcoma of the epididymis

ABSTRACT

Introduction: Leiomyosarcoma of the epididymis is a malignant tumor that is part of the mesenchymal origin´s paratesticular tumors with smooth muscle differentiation. Its location in the epididymis is very rare and represents less than 5% of the total of the genitourinary sarcomas.
Objective: To describe a case of leiomyosarcoma of the epididymis, the first in its kind that has been reported in Ciego de Ávila province.
Clinical case: 72-years-old masculine patient, white skin, with no history of urological disorders that attends to consultation due to the increase in the size of the left testicle and the presence of inflammation´s phlogistics signs accompanied by pain. On physical examination, it was found hard, stony and thickened epididymis. Complementary tests were normal. An scrotal orchiectomy was performed and it was tied the spermatic cord, as the treatment did not solve the problem. The histopathological study showed a leiomyosarcoma of epididymis.
Conclusions: The leiomyosarcoma of epididymis is a quite infrequent tumor. Surgery is the treatment with curative chances. Adjuvant treatments such as chemotherapy and radiation therapy may be useful in avoiding the local recurrence and in patients with metastatic disease.
