Complications of chronic urinary schistosomiasis

ABSTRACT

Introduction: Urinary schistosomiasis is endemic to Southern Africa. In its chronic form, tissue reactions affect the urinary bladder and the pelvic segments of the ureters. It can cause renal failure, squamous cell carcinoma, even death. Patients with chronic parasitic lesions were studied.
Objective: To identify chronic complications presented by patients.
Methods: A retrospective study was conducted in eight patients treated for five years due to chronic complications of the urinary schistosomiasis. The most outstanding parameters were analyzed.
Results: The average age of the patients was 37 years. Males and the age group between 32 and 47 years were predominant. Gross hematuria and the hypogastrium or lumbar pain prevailed. 75% of the patients were diagnosed with hydronephrosis, while bilateral ureteral calcifications and bladder calcifications affected the 33% and 56% of the cases, respectively. All presented vesical parietal thickening. The invasive squamous cell carcinoma of the bladder was treated in half of the patients. Three total cystectomy with ileal bypass and four vesicoureteral re-implantations were conducted.
Conclusions: The chronic form of schistosomiasis can be asymptomatic and is uncommon in our hospital. Urinary tract obstructions cause different degrees of kidney failure, which may improve after surgical treatment. Patients with bladder´s squamous cell carcinomas attended in advanced stages of the disease and in all of them was confirmed the presence of urinary schistosomiasis.
