Replacement of ports by percutaneous needles in endoscopic surgery

ABSTRACT

Objective: To present a new model of operation in endoscopic surgery with percutaneous needles of 1 mm diameter, which replace ports that assist in traditional laparoscopy.
Introduction: Endoscopic surgery is a method which considerably reduces invasion to a minimum extent tissue; it’s considered to be a gold standard because of its results in some pathological surgeries. The tendency in recent years of reducing the size of instruments and ports to optimize the results of traditional laparoscopic surgery has resulted in the creation of miniinstruments and minilaparoscopic appliances.
Methods: Our group performed from March 1997 to September 2003, 622 procedures of laparoscopic surgery, with one umbilical port (500 cholecystectomies, 62 hysterectomies, 48 appendectomies, 12 ovarian cystectomies). Percutaneous needles (hook needle, passing suture needle, threading needle, atraumatic straight needle calibre 2-0) replace the system ports and some instruments as an effective way when they are used in laparoscopic surgery for diverse pathologies.
Results: Percutaneous needles effectively fulfilled functions of traction, mobility, dissection and helping when placing the stitching material, allowing to finalize the procedure with a single umbilical port in 85% of the cases. Improve the aesthetic and functional results, beside to reduce costs, when they are compared with traditional laparoscopic surgery.
Conclusion: When one has the experience in the use of percutaneous needles for laparoscopic surgery, it is possible to use them by replacing the assisting ports as well as instrumentation in some endoscopic procedures, optimizing the aesthetic and functional results.
