Intrahepatic cholestasis of pregnancy: a systematic review

ABSTRACT

Intrahepatic cholestasis of pregnancy is the most common pregnancy specific liver disease that is typically presented in the third trimester.The etiopathogenesis of intrahepatic cholestasis of pregnancy includes genetic and environmental factors. It occurs because bile acids in the maternal and fetal blood increase, causing itching in the mother and increased risk of adverse perinatal outcomes, including spontaneous preterm delivery, meconium staining of amniotic fluid, and fetal death.
Diagnostic confirmation is made with an increase in bilirubin in maternal blood and is treated with ursodeoxychiolic acid, although it does not reduce perinatal morbidity and mortality.
The studies available today for the assessment of fetal well-being are not useful to predict the risk of fetal death, since it seems clear thar this occurs due to sudden acute fetal hypoxia. The objective of this writing is to present an update review of the etiology, diagnosis and management of this disease.
