Sites of obstruction in obstructive sleep apnea syndrome evaluated with VOTE scale and influence on multilevel surgical selection

ABSTRACT

Objective: To describe and compare the sites of obstruction in the upper airway observed by endoscopy without and under sedation using the VOTE scale and assess its influence on the selection of surgical procedures in patients with obstructive sleep apnea syndrome who underwent multilevel surgery.
Materials and Methods: A retrospective, cross-sectional, analytical observational study of patients with obstructive sleep apnea syndrome undergoing multilevel surgery from January 2013 to March 2020.
Results: Thirty-two patients were included. The degree of obstruction was significantly more severe in practically all types and locations evaluated in the endoscopy under sedation. The initial surgical plan was modified after endoscopy under sedation in 21/32 patients. This difference was statistically significant.
Conclusions: Endoscopy under sedation is a dynamic, inexpensive, safe and easy to perform study, it helps to identify the true sites, types and degree of obstruction, which could be underestimated during a normal endoscopy. The initial surgical plan is modified in a significant percentage after performing the endoscopy under sedation due to a more accurate visualization.
