ABSTRACT

The diagnosis and treatment of sinoatrial dysfunction is a common problem in practice; it is made up of various rhythm disorders that can lead to major cardiovascular events. It includes intrinsic structural abnormalities of the node, its dysfunction, hereditary causes, drug effects, and pathological autonomic influences. More than an isolated entity, it is a spectrum of symptoms and arrhythmias, with abnormalities in the formation and propagation of the electrical impulse, which by various pathophysiological mechanisms lead to similar phenotypes of the disease. Its diagnosis is clinical and paraclinical. We present a patient with contradictory arrhythmias: sinus bradycardia and atrial flutter with 5: 1 and 1: 1 atrioventricular block. Pharmacological therapy and two flutter ablations were unsuccessful and a third successful There were episodes of asystole, prolonged PR interval and complete atrioventricular block, a permanent pacemaker was implanted. This entity should be assessed as part of a disease of the conduction system (similar causes and mechanisms at different levels, frequent associations), with a predominance in one or the other structure.
