Small intestinal bacterial overgrowth in clinical practice

ABSTRACT

Small intestinal bacterial overgrowth correlates with the presence of ›105 CFU/ml of bacteria in the proximal small intestine and/or colonization by abnormal bacteria. The prevalence in the general population is still unknown, ranging between 0% to 15.6% in healthy individuals according to different sources. This clinical entity is characterized by alterations in gastric acid secretion, intestinal integrity and motility, and proteolytic enzymes. Clinical presentation is derived from the malabsorption of metabolites and its nutritional consequences in addition to its systemic inflammatory effects. Diagnosis is made by quantifying bacteria in a small intestine aspirate, or indirectly by a hydrogen breath test. Management of small intestinal bacterial overgrowth must be individualized and include treatment of the underlying disease, nutritional support, and the use of short courses of selective antibiotics.
