ABSTRACT

Spinal epidural lipomatosis (SEL) is a rare disease, characterized by overgrowth of epidural adipose tissue within the spinal canal. Its causes include: exogenous steroid use, endogenous steroidal hormonal disease, obesity, surgery-induced and idiopathic SEL. Furthermore, established kyphotic deformity has been also postulated as another possible cause. A 15-year-old man with pain in the thoracic spine of mechanical characteristics and without warning signs was studied. On physical examination, a kyphotic attitude and D2-D6 apophysalgia standed out. Given the persistence of pain despite the prescribed rehabilitation treatment, magnetic resonance imaging (MRI) was performed, observing posterior epidural lipomatosis from D3 to D9. Because there is a possibility that the size of the SEL may increase leading to neurological symptoms, follow-up of the patient is recommended, even though the patient is currently asymptomatic.
