Primary hyperparathyroidism due to a parathyroid carcinoma associated with papillary thyroid carcinoma

ABSTRACT

Hyperparathyroidism is a disease caused by the increased secretion of the parathyroid hormone. A 46-year-old male patient went to the Endocrinology consultation at the People´s Committee Center of Medical Specialties, Quito, Ecuador, presenting initially an asymptomatic hypercalcemia with persisting calcium values, between 13.9 and 15.2 mg/dl. He also admitted having bone pain of different intensity since 6 months before, located in the forearms and the right leg. On the physical examination, the patient´s weight was 81 kg, his height was 178 cm, the body max index was 25.5 kg/m2 and his blood pressure was 140/80 mmHg. The related symptoms and signs, as well as complementary examinations, led to the clinical diagnosis of a primary hyperparathyroidism due to a parathyroid carcinoma, associated to a papillary thyroid carcinoma and a chronic renal failure. This case was considered appropriate for the report, due to the atypical manifestations and the absence of similar reports in the reviewed medical literature.
