Plasma cell myeloma, clear cell variant with expression of IgM and numerous asteroid B-cells

ABSTRACT

Background: The cytomorphology of plasma cell neoplasms can vary from indistinguishable forms of mature normal plasma cells to immature plasmablastic, pleomorphic or small cells, among others. These unusual forms may lead to an erroneous diagnosis.
Clinical cases: A 73-year-old male patient with plasma cell myeloma with clear cell features expressing CD138/IgM/kappa. The bone marrow biopsy also showed numerous asteroid-shaped B cells with dendritic cytoplasmic processes. These B lymphocytes expressed CD20 and CD79a and had the features of thymic asteroid B cells.
Conclusions: This is the first report of a plasma cell myeloma with asteroid B cellrich microenvironment in bone marrow. Due to the close association between these two cell populations, neoplastic plasma cells might depend upon asteroid B cells, but this remains to be determined.
