Lupus Erythematosus Profundus in a Young Man: Case Report

ABSTRACT

Introduction: Lupus erythematosus (LE) is a rare autoimmune disease in males that presents multiple clinical manifestations related to its prognosis. The etiology is unknown, nevertheless it is related to ultraviolet radiation exposure and modification in lymphocytic function. Lupus erythematosus profundus (LEP) is the clinical manifestation found in only 1% to 3% of patients with cutaneous lupus erythematosus (LEC) and only 10% of the cases reported in the literature are males. It is recalcitrant to treatment with frequent exacerbations. Up to 50% of patients with LEP progress to systemic lupus erythematosus (SLE) during the course of the disease.
Case: We present the case of a previously healthy 16-year-old man who presented with an indurated plaque on the left cheek with a chronic course. The incisional biopsy revealed LEP. Work up analysis found no systemic lupus erythematosus involvement. The patient received topical corticoid treatment and strict photoprotection with improvement of the erythema and reduction of the indurated area.
Conclusion: LEP is a rare dermatosis in males, so the current literature may not be representative in this sex. It is necessary to carry out a comprehensive approach at diagnosis with serum and rheumatological laboratory tests. It is of outmost important to be able to identify early signs and symptoms that suggest progression to SLE or lymphoma.
