Support for quality of life in patients with osteosarcoma. Experience of the pediatric palliative care department

ABSTRACT

Osteosarcoma is the most common bone tumor in childhood ranking sixth in frequency; it is extremely aggressive and survival in Mexico is low. It causes complex symptoms that deteriorate the quality of life. Previously palliative care focused on end-of-life care, now it begins diagnosis by improving adherence to treatment, improving symptoms and directly increasing survival. Given this reality, the objective of this study involved the description of the characteristics of pediatric patients with osteosarcoma who have been seen in the quality of life support department. The design was retrospective and descriptive. Through the diagnostic record of the clinical archive, the records of patients with osteosarcoma were obtained. We included 36 cases, distribution by sex H:M, 60:40, with a mean age of 12 years. Pain (77.7%) and dyspnea (47.2%) were the most frequent clinical manifestations. 38.8% of patients were referred to a curative phase of the disease; 30.5% in incurable stage with poor quality of life. Regarding survival, only 1 patient is still alive (2.7%). The results are consistent with other studies in national and international hospitals. Pain and dyspnea are the most frequent clinical manifestations, so identifying and treating them is a priority. The use of opioids is warranted for the symptomatic management of patients with osteosarcoma. Quality of life support services need to be included in the initial care of all patients diagnosed with cancer.
